The only way to make or change a beneficiary designation is to submit a properly completed Beneficiary Designation Form to the Fund Office before the date of your death. This official form not only designates beneficiaries for benefits payable under the Christmas Bonus Plan but for other SCPT Plans.
TIP: Be sure to send a properly completed Beneficiary Designation Form to the Fund Office whenever changes affect your family status or named beneficiaries.
