Change in Marital Status

You must promptly notify the Fund Office if you marry or divorce by providing an original, official marriage certificate or documentation of divorce. (Church or souvenir marriage certificates are not acceptable.). You should also complete an Enrollment and Beneficiary Form (available from your local union office or the Fund Office, or by downloading and printing the form on this web site. Note: The Plan recognizes same-sex marriages and most domestic partnerships upon receipt of proper documentation.
