Your Health and Welfare Fund provides you with high quality and affordable vision benefits administered by Anthem Blue Cross Blue View Vision.
New participants may enroll for vision coverage by sending a completed Blue View Vision Enrollment Form to the Fund Office within 60 days of the date when first eligible. After the first 60 days, your vision coverage becomes effective the month following the date when the form is received, not retroactive to your initial eligibility date.
Contact Anthem Blue Cross to verify Blue View Vision coverage before scheduling vision care. Once you are enrolled, you can make vision care appointments with your Blue View Vision network providers for you and your covered dependents.
You may choose any licensed eye care professional, but the paid benefit amounts will be greater when you use Blue View Vision network providers. The network provider will collect any copayments at your visit and bill Anthem Blue Cross directly. There are no claim forms to file.
If you choose providers outside of the Blue View vision network, you only get an allowance toward services and must pay the rest. (In-network benefits and discounts will not apply.) Your out-of-pocket costs may be higher. You must also pay in full at time of service and submit claims/receipts directly to Anthem Blue Cross for reimbursement.
