When you become eligible, the Fund Office sends you a welcome package that includes the following forms you must complete and return to the Fund Office within 60 days:

Landscape Trust Enrollment and Beneficiary Form (PDF)
Western Growers Assurance Trust Enrollment Card (PDF)
Delta Health Services Enrollment Form (PDF)
Blue View Anthem Blue Cross Vision Enrollment Form (PDF)
The Hartford Life Beneficiary Designation Form (PDF)
You may also download these forms at the links above or by contacting the Fund Office. You cannot be enrolled for benefits until the Fund Office receives all the properly completed forms.
If you don’t enroll by the 60-day deadline, you won’t have coverage until a future date.
