You may make or change a beneficiary designation at any time by completing, before the date of your death, a properly completed Enrollment & Beneficiary Form. The beneficiary designation takes effect when the signed form is received by the Fund Office. The Hartford Life Insurance benefits have a separate beneficiary designation form that must be completed upon enrollment.
If you do not designate a beneficiary or if the beneficiary predeceases you, the Plan pays benefits in this order:
Your Enrollment & Beneficiary Form names your beneficiary under the Health & Welfare Fund, Defined Contribution Pension Fund and the SCPT Defined Contribution Fund.
