When you complete your Enrollment and Beneficiary Form, you will designate your beneficiary or beneficiaries to receive your individual account balance following your death. The form may be obtained from your local union office or the Fund Office or by downloading and printing the form on this web site.
If you are married at the time of your death and have not designated a beneficiary, your spouse is automatically your beneficiary. If you designate your spouse on the Enrollment and Beneficiary Form and later divorce, your former spouse will remain your beneficiary unless you submit a change to the Fund Office using the Enrollment and Beneficiary Form.
